The truth is, whether you’ve dropped unwanted pounds through diet and workouts or with weight-loss medications, there’s no miracle moisturizer, skin-tightening device, or exercise that can tighten loose, sagging skin that has lost its elasticity after major weight loss. That’s because skin without elasticity can’t be tightened from the outside in; it has to be surgically removed, which is where body contouring procedures come in.

Which Procedures Fix Sagging Skin After Weight Loss?
Surgical skin removal is the only reliable way to fix sagging skin after major weight loss. While liposuction can help target and sculpt stubborn areas of fat, it doesn’t remove excess skin on its own. Depending on where you’re carrying excess skin, I may recommend one or more of the following customizable body contouring procedures:
Arm Lift
An arm lift (brachioplasty) removes excess, hanging skin from the upper arms, often called “bat wings.” I tighten the underlying tissue and reposition the skin so your arms look sleeker from shoulder to elbow, even when you raise them.
Breast Lift
A breast lift (mastopexy) raises and reshapes drooping breasts after weight loss. I remove excess skin and reposition the breast tissue higher on the chest wall, restoring a more youthful contour. For patients who also want fuller breasts, I perform a breast lift with implants. Alternatively, some women benefit from breast reduction and men choose gynecomastia surgery to balance and complement their sleeker frame.
Tummy Tuck
A tummy tuck (abdominoplasty) removes excess skin from the abdomen and tightens the underlying muscles for a firmer, flatter midsection. It’s often the right choice for patients whose skin laxity is concentrated at the front of the abdomen rather than wrapping around the flanks and lower back.
Thigh Lift
A thigh lift removes excess skin from the inner or outer thighs (saddlebags). I tailor the incision pattern to your specific concerns and the degree of skin laxity, whether that’s isolated to the inner thigh or extends toward the hip.
Body Lift
A body lift is the most transformative option to address sagging skin on the upper and lower body, including the abdomen, flanks, lower back, and buttocks. While it’s a major surgery with multiple incisions and a substantial recovery period, many patients appreciate the opportunity to address multiple problem areas in a single comprehensive procedure.
Fat Transfer
Fat transfer uses your own fat, harvested through gentle liposuction, to restore volume in areas left deflated by weight loss. I often use this technique to soften contour irregularities or add fullness to the buttocks, breasts, or face after skin removal surgery.
Brazilian Butt Lift
A Brazilian butt lift (BBL) uses fat transfer to restore volume and shape to the buttocks, which often lose fullness after significant weight loss. I harvest excess fat through liposuction, then strategically place it to create a natural, proportional silhouette.
Lipedema Surgery
Lipedema surgery removes the disproportionate fat deposits caused by lipedema, a condition that diet, exercise, and weight loss alone can’t resolve. It uses specialized liposuction techniques that work with your lymphatic system to reduce the fat responsible for swelling and discomfort.
Facelift
A facelift is the best solution for addressing “Ozempic face,” the hollow, deflated look that can develop after rapid weight loss. I tighten loose facial skin, restore lost volume, and reposition underlying tissue to refresh your contours without looking pulled or overdone. I frequently pair this with neck rejuvenation for patients whose weight loss has also left them with loose skin along the jawline and neck.
Most of my post-weight loss patients benefit from staging 2 or 3 procedures over several months, so we can prioritize the areas that affect their quality of life first.
Body Lift vs. Tummy Tuck: Which Do I Need?
A body lift addresses more surface area than a tummy tuck, making it a more comprehensive choice after major weight loss. A tummy tuck focuses on the abdomen alone, while a body lift extends that same skin removal circumferentially, encompassing the abdomen, lower back, and flanks. This approach is typically necessary once you’ve lost 50, 100, or more pounds and have skin laxity that wraps around your entire midsection. Body Lift vs. Lipo 360: What’s the Difference?
Lipo 360 removes fat; a body lift removes skin. After significant weight loss, most patients need the latter. Lipo 360 effectively suctions fat from the abdomen, waist, and back, but it can’t tighten skin that’s already lost its elasticity. That’s why I often pair the procedures: liposuction to refine stubborn fat pockets, combined with a body lift to remove the excess skin that liposuction alone can’t resolve.
